Leg Length Discrepancy

I came across your website while researching leg length discrepancy. I’m an amateur cyclist and runner from Atlanta, GA. I was involved in a cycling accident in August of 2009 where I suffered an intertrochanteric hip fracture. I had surgery to repair the fracture resulting in an implant through the femoral neck and the femur. I had the implants removed in March of 2011. Since the initial accident I’ve experienced a shorter leg length on my injured (right) side (and accompanying hip/leg/lower back pain). I’ve seen multiple chiropractors and physical therapists about this issue. It has gotten better, but no one has been able to completely correct the problem. A few weeks ago, my surgeon took vertical x-rays of my lower body and through rudimentary measurements determined I have a 4mm to 6mm actual leg length discrepancy and a 10mm to 12mm functional discrepancy. He has scheduled me for a CT scan later this month to determine exactly what the actual discrepancy is. To me it seems to have more to do with hip rotation, because the discrepancy changes between lying and sitting. Can the functional leg length discrepancy be corrected? Do you know a true expert in the Atlanta area that can help me with my issue? Although 1cm to 2cm doesn’t sound like much this issue have severely hampered my athletic ability and affected my quality of life. Thanks in advance for your help.

Difficult Situation

Sorry for the delay in my response.

Your situation is obviously very complicated and it is tough for me to give you an exact answer with limited information. But, given what you have told me, I would use a combination of treatments to correct your problems.

First, I would correct your anatomical leg length discrepancy with a heel lift that matches the amount of our issue. So, if they determine that the anatomical difference is 3mm, you need a 3 mm lift. That needs to be worn all of the time.

From there, you need to find someone who is good and experienced with fixing your rotation. This person needs to approach it with a combination of manual soft tissue work, mobility, strengthening and actual joint mobilization/manipulation. There are some chiropractors who use all of these techniques and some PTs or ATCs who use them. They are kind of rare, so you need to work hard and do your homework. Unfortunately I don't have anyone in the Atlanta area that can help you. You may need to work with several people in a team approach. The only downside to that is figuring out who is the "coach" of the team and decides when you go where and get what treatments.

Hopefully once you get a lift in, you can make a change in the mechanics at your hip and fixing the functional leg length issue more easily.
